How can I make custom field clickable?

We have a custom field which we use to store a "Software Request ID" just as free text. It is usually a 5-digit number.

I'd like to use the value of this field and be able to click on it to go to a web page on another server with the value of the field as a URL parameter.

So, for instance, if the field contains "12345", I want to be able to click the field and the browser goes to "http://server/some/path?ID=12345".

Is this possible?

Not out of the box. A relative question has been brought up like this here (https://answers.atlassian.com/questions/130437). There are several pointers in there, choose what you can implement easily.
