With Crucible/JIRA integration, when closing a review, the user is given the option to resolve the JIRA ticket.
Is there a way to modify that form to include changing the JIRA Assignee field?

According to the documentation, this is not possible at the moment. Please refer to this feature request: https://jira.atlassian.com/browse/CRUC-6500
If you want to do that just for a single project - you have to create for that project (or modify the exisiting one) workflow schema, create resolve screen and apply it to the state on the schema when issue is resolved.
If you want to do that for all the projects, just go to Admistration -> Screens, find Default Resolve Screen and click "configue" and modify it as you need.

It's getting Crucible to see that and use it. It only hows the resolution and status field.
